We believe that patient exposure to intensive preventative education can significantly decrease the complications associated with dialysis initiation and slow the progression to ESKD. Our joint program can extend your capability to help patients without additional costs to your practice. The program is consistent with healthcare trends toward population health management and value-based care. 

Services provided by MKC:

• Tailored kidney education with an Advanced Practice Registered Nurse and Care Manager
• One-on-one medical nutrition therapy with a Registered Dietitian
• Care coordination and health coaching
• Psychosocial support provided by a master’s prepared Licensed Social Worker
