The Patient Centered Medical Home (PCMH) model emphasizes coordinated team based care to achieve:

– Lower costs per capita
– Better health for the population
– Improved patient experience of care
– Cultivating joy of practice

Choosing PMAG as your PO for PCMH transformation provides physicians and their practices with concrete guidance to obtain sustainable practice transformation. Partners for Quality Health (PQH) provides individualized attention for physicians to obtain PCMH recognition and helps maximize financial potential through PCMH and insurance company Pay For Quality (P4Q) programs.

The QCIPN allows physicians to connect with each other and collaboratively design physician led systems of care. PMAG sponsors PQH services to keep you up to date on requirements and provide support to transform your practice.
